An accident report is required in the name of the individual / concerned entity.

Procedures

1. Visit the concerned Directorate or Operations Department.
2. Fill out the application form to get the accident report.
3. The relevant officer in the Operations Department shall check the application.
4. To hand-over the applicant an official letter and a certified copy of the incident report.

Required Documents

• Proof of identity.
• A formal authorization from the party concerned if the applicant is another person.

Any individual or party that has had an accident and has been treated by the Civil Defense.
